The Jember Regency Government, through the Health, Population Control and Family Planning Office (Dinkes P2KB), has officially introduced the application flow for proactive health services or home care. This program is specifically designed to facilitate medical access for vulnerable and underprivileged community groups in the region without having to visit health facilities directly.
The Head of the Jember Regency Dinkes P2KB, Muhammad Zamroni, explained that this service is scheduled to launch officially on July 24, 2026, at the Sumberbaru Puskesmas. The inauguration event is planned to be attended directly by the Vice Minister of Health. To access its benefits, the community can register through two main channels, namely the Wadul Gus’e complaint platform or by contacting the emergency number (hotline) of the nearest Puskesmas.
The administrative requirements that prospective applicants need to prepare include an Identity Card (KTP), Family Card (KK), an active mobile number connected to WhatsApp, and previous medical records, if any. After the documents are submitted, the medical team will carry out a screening and factual verification process to ensure this medical assistance is well-targeted.
This program prioritizes patients from underprivileged families with chronic illnesses, stunted toddlers, high-risk pregnant or postpartum mothers, and people with disabilities. Patients who pass verification will receive regular home visit schedules. The patients' entire medical history and progress will later be integrated into the Health Information System (SIMKES) for continuous digital monitoring.
In its implementation, the deployed field team consists of professionals ranging from nurses and midwives to nutritionists carrying essential medicine logistics. In addition to direct medical action at the patient's home, this service is also strengthened by remote communication features (telemedicine), allowing the field team to consult directly with general practitioners at the Puskesmas or specialists at referral hospitals during emergency situations.
